We understand how intimidating it can be to look at a towering stack of paperwork and wonder, “Where do I even start?” Our Case Management team has been navigating the system long enough to know exactly where to start and how to use our knowledge and resources to arrive at the best outcome for you!

We can help with:

  • Advance directives / medical power of attorney
  • Arranging check-in visits to ill or aging patients
  • Assistance with completing applications for:
  • Eldercare
  • Healthcare management
  • Identifying equipment needs or home modifications necessary for accessibility
  • Risk management
  • Screening/assessments

United in the Fight Grant

United in the Fight is a program we offer to any Wyoming Resident with any type of cancer. The program offers navigation, case management, and mental health services to anyone impacted by cancer at no cost.

Veteran Directed Care

Veteran-directed Home and Community-based Services is a program funded by the VA that allows eligible Veterans to receive services in their home and community while managing their own service plan and budget.

Community Choices Waiver

CCW is a Medicaid program that provides long-term care services for individuals who require a nursing home level of care, but choose to remain living in the community rather than in a nursing home.

Three Options for Visits

At LIV Health, we offer three ways to deliver our Mental Health, Medical, Case Management and Community Caregiving services to our patients; telemedicine, at your home, or in one of our office locations. Insurance Accepted

Medicare, Medicaid, all commercial insurances accepted, workers compensation, and a sliding fee scale offered for individuals without insurance. If you have any questions, please contact us.
