United in the Fight Grant

United in the Fight is a program we offer to any Wyoming Resident with any type of cancer. The program offers navigation, case management, and mental health services to anyone impacted by cancer at no cost.

Veteran Directed Care

Veteran-directed Home and Community-based Services is a program funded by the VA that allows eligible Veterans to receive services in their home and community while managing their own service plan and budget.

Community Choices Waiver

CCW is a Medicaid program that provides long-term care services for individuals who require a nursing home level of care, but choose to remain living in the community rather than in a nursing home.

Comprehensive and Supports Waiver

DDW is a Medicaid program that provides supports and services to individuals with an intellectual and or developmental disability.

Three Options for Visits

At LIV Health, we offer three ways to deliver our Mental Health, Medical, and Case Management services to our patients; telemedicine, at your home, or in one of our office locations. Insurance Accepted

Medicare, Medicaid, all commercial insurances accepted, workers compensation, and a sliding fee scale offered for individuals without insurance. If you have any questions, please contact us.
