How to Make a Referral
Making a referral doesn’t have to be hard or time-consuming.
We have tried to streamline our process and make it as simple as possible! So let’s talk about how to make a referral, who can make a referral, and what goes on behind the scenes.
First up, HOW TO MAKE A REFERRAL…
Making a referral to LIV Health is pretty simple, it’s as easy as 1, 2, 3.
Step 1. Fill out our simple referral form.
Our referral form is short and sweet; only one page and can be found here.
If you do not have access to a computer, aren’t great at navigating the world wide web, or would just rather call and do it over the phone, that’s ok too. We’re happy to help you fill it out, just give us a ring at 307.630.4729.
Step 2. Fax, mail, email or drop off your completed referral form.

We do have a physical address too (415 W. 27th Street in Cheyenne), however, since we see clients in their homes there is not always someone available, so call 307.630.4729 or email before stopping by, we would be happy to schedule a time to meet you there!
Step 3. Schedule an appointment.
One of our LIV Health team members will call you to set up your first appointment! No office visit required, we will come to you!
Next up: Who can refer?
Anyone! We get our referrals from everywhere: nursing homes, physicians, dentists, law offices, friends, family, veterinarians, assisted living facilities, other care providers, other mental health professionals…the list could go on and on. But, most importantly you should know you don’t have to have a referral from any of those places. While we absolutely appreciate those referrals, it’s not mandatory, you can refer yourself!
The process: What happens after we’ve received your referral?
Our process behind the scenes runs like a well-oiled machine.
Step 1. We receive the completed referral form, well usually Kaila receives your form. Hey Kaila! She get’s your information entered into our system, and sends you out one of our welcome packets.
Step 2. It goes to Wendy, our Referral Coordinator/Lead Case Manager/In House Rock Star, who assigns you to one of our awesome case managers, therapists or psychologists.
Step 3. Your case manager/therapist/psychologist will reach out within 3-5 business days to get you scheduled for your first appointment…it’s as easy as that.
As we mentioned above we usually see people in their homes, but we can also see people in the community or at one of our convenient office locations, we don’t want transportation or living situation to be a barrier, so we have tried to make it as convenient as possible for you and your case manager/counselor/psychologist to meet!
