ABOUT CASE MANAGEMENT...
What if I'm not insured? Will you still work with me?
Yes! As a community resource, we do our best not to turn any patient away. We can even provide you with education and assistance on health care/insurance options. LIV HEALTH will offer a Sliding Fee Discount Program to all who are unable to pay. LIV HEALTH will base the program eligibility on a person’s ability to pay and will not discriminate on the basis of age, gender, race, sexual orientation, creed, religion, disability, or national origin. The Federal Poverty Guidelines will be used in determining eligibility. This policy will be reviewed on an annual basis and updated accordingly with the Federal Poverty Guidelines.
What if I don't understand the paperwork?
Don’t worry; we do! We have over a decade of experience navigating the healthcare and community system, and we’ll be there for you every step of the way. Not only will we help you fill out all your paperwork, we’ll also make sure you understand the process, what you are signing, and what to expect after turning it in.
Will you force me or my loved one to go to a nursing home?
No! We will not force anyone to do anything. Our first step is always to evaluate your needs and determine if services can be put into place to help you stay in your home. We may recommend a higher level of care if you are struggling at home, and if that’s the case, we can find the resources needed to begin a care plan for you.
Will you provide me or my loved one with transportation?
Unfortunately, no, not at this time. However, we will explore all options in cases where transportation is needed, including accessing community resources.
What if I can't get a home health agency to work with me?
We can be your advocate and help you access care! We have years of experience and knowledge working with complex medical systems, and we understand the requirements and resources needed for home health.

Will you keep the information I provide private?
