What medical services do you provide at LIV Health?
Bridging the Gap: We are here to help bridge the gap by providing in-home or telehealth medical appointments by a physician or nurse practitioner when you are unable to get a timely appointment with your primary care provider. We provide well-rounded care for our clients and ensure your primary care provider receives our visit summaries. Our providers are experts in geriatrics, acute illnesses, changes in condition, chronic illnesses, care coordination, advanced care planning, and dementia screening and support.
Psychiatric Services: We offer psychiatric medication management support including diagnosis and evaluations. We work alongside your mental health team to develop a plan of care that supports you.

CARE COORDINATION
Care Coordination means that we take time to organize client care activities and share information with all of the participants involved with a patient’s care to achieve safer and more effective care.
SYMPTOM MANAGEMENT
Our goal with symptom management is to prevent or treat, as early as possible, the symptoms of a disease, side effects caused by treatment of a disease, and psychological, social, and spiritual problems related to a disease or its treatment. This type of treatment is also called supportive care.
PSYCHIATRIC MEDICATION EVALUATION
Psychiatric medication evaluations may be used to diagnose problems with memory, thought processes, and behaviors. Diagnoses can include depression, schizophrenia, anxiety, bipolar disorder, addiction, and more.
A psychiatric evaluation aims to:
- Include the patient while making decisions about an initial treatment plan
- Rule out other physical conditions that might be causing the symptoms
- Identify long-term problems that might emerge in the future
- Make changes in the treatment if needed, for those who have had a psychiatric evaluation in the past
- Confirm the diagnosis of a mental disorder that requires treatment by a psychiatrist
DEMENTIA SCREENING AND SUPPORT
Our providers are experts in Dementia and are happy to help screen, provide education and resources, and provide support to the person with Dementia and their families/caregivers.
ADVANCED CARE PLANNING
Advanced Care Planning is a process that supports adults at any age or stage of health in understanding and sharing their personal values, life goals, and preferences regarding future medical care.
CHRONIC CARE MANAGEMENT
Chronic Care Management (CCM) is a Medicare and Medicaid program that is available to help patients with chronic illnesses and health conditions. These conditions may include diabetes, high blood pressure, heart disease, depression, multiple sclerosis, sleep apnea, and more. CCM can help you to learn and understand your conditions and live successfully with them. The work involves motivating, intervening, and helping to achieve an improved quality of life. REMOTE PATIENT MONITORING
Remote Patient Monitoring uses digital technologies to collect medical and other forms of health data from individuals in one location and electronically transmit that information securely to health care providers in a different location for assessment and recommendations.
