Here are some of the BEST things that have happened at LIV this year…

VETERAN DIRECTED CARE in collaboration with the Center for People with Disabilities in Boulder. This program allows eligible Veterans to receive services in their home and community while managing their own service plan and budget. Veteran-directed care helps veterans of all ages who need non-medical supports and services, coaching and assistance with daily or instrumental activities, are isolated or whose caregiver is overburdened.

UNITED IN THE FIGHT GRANT in partnership with Wyoming Breast Cancer Initiative & Stitches Acute Care Center where we provide mental health support for those affected by Breast Cancer and their families.

UNITED IN THE FIGHT GRANT in partnership with the Wyoming Department of Health where we provide navigation services for those affected by Breast Cancer and their families.

GERIATRIC WORKFORCE ENHANCEMENT PROGRAM (GWEP) in partnership with Ivinson Medical Group in Laramie brings chronic care management to Albany County.
