WHO WE ARE
LIV Health is a team of caring, experienced mental health and medical professionals, working one-on-one with our patients to navigate paperwork, create care plans, provide counseling and more.
What We Do
LIV Health brings all elements of case management, counseling, medical and supportive services directly to our patients’ homes or their nursing home/assisted care facility.
How We Do It
Every situation is unique, and no solution is one-size-fits-all. Here’s how to get started with LIV Health, whether it’s mental health counseling, medical services, case management, or community caregiving.
Getting started with LIV Health is easy
No office visits required. No long and tedious intake process. Just you and your personal mental health counselor, sitting down together in the comfort and convenience of your home.
We offer Gap Care for medical services when the patient is not connected to primary medical care or when care is not otherwise available. We seek to connect patients to and collaborate with primary care providers.
We understand how intimidating it can be to look at a towering stack of paperwork and wonder, “Where do I even start?” We’ve been navigating the system long enough to know exactly where to start and how to use our knowledge and resources to arrive at the best outcome for you!
Rely on us to ensure you or your loved ones have the support needed to live independently! In addition to in-home services, we provide concierge (helper) services, and transportation services, too!
The LIV Health Difference
